Elephas Laboratories (“Elephas,” “we,” “our,” or “us”) is committed to protecting the privacy and security of your protected health information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and applicable state laws. This Notice describes how we may use and disclose PHI when providing our laboratory services—including the Elephas eLIVE laboratory developed test (“eLIVE”)—and explains your rights regarding your PHI.
We may use or disclose your PHI for purposes including, but not limited to:
Treatment: To perform the eLIVE test and report results to your authorized healthcare provider.
Payment: To bill and obtain payment for laboratory services, as permitted.
Healthcare Operations: For activities necessary to support quality, safety, accreditation, regulatory compliance, and overall laboratory operations.
We may also use or disclose your PHI without your written authorization in other situations permitted or required by law, such as public health reporting, law-enforcement requests, and responding to court orders.
Any other use or disclosure of your PHI will require your prior written authorization. You may revoke such authorization at any time, except where we have already relied on it.
You have the following rights under HIPAA:
Right to Access: You may request a copy of the PHI we maintain about you.
Right to Amend: You may request corrections to your PHI if you believe it is inaccurate or incomplete.
Right to Restrictions: You may request limits on how we use or disclose your PHI.
Right to Confidential Communications: You may request that we communicate with you through specific means or at alternate locations.
Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
Right to a Paper Copy: You may request a paper copy of this Notice at any time.

Elephas Laboratories is required by law to:
Maintain the privacy and security of your PHI
Provide you with this Notice describing our legal duties and privacy practices
Notify you if a breach occurs that compromises the privacy or security of your PHI
Follow the terms of the Notice currently in effect
We reserve the right to update or revise this Notice at any time. Any changes will apply to all PHI we maintain and will be posted on this website.
